Insilico Medicine and CBRM announce a licensing agreement


In 2013 CBRM, a research organization headquartered in Moscow, Russia, sponsored a multi-center study, where multiple in silico predictions generated by scientists at Insilico Medicine were validated on in vitro on senescent cells and analyzed in multiple imaging and microarray experiments. A range of predictions with minimal or no side effects rescued the senescent phenotype and lead to transcriptomic signatures resembling "young" non-senescent cells. Insilico Medicine will receive the research results prior to publication in order to explore synergistic and cumulative effects of these compounds and perform further validation and compounding with the full rights to file for intellectual property and sub-license the findings.
 
The Center for Biogerontology and Regenerative Medicine is supporting multiple multi-disciplinary studies in aging and longevity, organizing industry forums and seminars for young scientists. It is dedicated to reducing Russia's dependence on natural resource-driven economy by fostering a generation of young leaders in systems biology of aging and longevity and age-related diseases. The study validating the efficacy of molecules with predicted geroprotective properties in vitro was one of the landmark research projects of CBRM.

Insilico Medicine launched Aging.AI

Aging.AI system allowing users to guess their age and gender by entering the results of their blood test. The system will not be used for medical purposes at this time and is focused on gamification of consumer blood testing and attracting the attention of the general public to the importance of periodic blood tests.

The renaissance in deep learning, led by the teams of Geoffrey Hinton, Yann LeCun, Andrew Ng and several other thought leaders, is revolutionizing many areas of information technology in the same way that CRISPR/CAS9 is revolutionizing biomedicine. However, the propagation of deep learning into biomedicine has been slow. Since 2014, Insilico Medicine has been hard at work harmonizing millions of gene expression samples and developing deep-learned transcriptomic biomarkers of cancer, aging, and other diseases for clients worldwide, in many cases achieving performance superior to other machine learning methods. Aging.AI is not a transcriptomic marker, it is a side project of the company's collaboration with leading diagnostic centers providing fully-anonymized blood biochemistry and cell-count test data linked to age and gender of patients that are presumably free of life-threatening diseases other than aging. Almost a million samples were used to train an ensemble of deep neural networks to predict age and gender of the patient and deployed as a web-based tool, which can be used for entertainment purposes by the customers of diagnostic clinics and make blood testing more fun.

Aging is a disease and our target is to find ways to treat it or even cure it. Advances in deep learning and multi-omics integration will help us find actionable markers of aging in humans and develop novel interventions to extend healthy longevity.

"The launch of Microsoft's How-Old.net, which can predict the age of the person by the photograph, inspired us to develop a consumer-friendly system to guess the patient's age by simple blood biochemistry. Our company is quite good at developing similar tissue-specific biomarkers trained on large number of transcriptomic data sets in order to predict the geroprotective efficacy of multiple anti-aging, CNS, metabolic and anti-cancer drugs. But here we stepped outside our core competence. Users may find it interesting to see if the predicted age changes after a certain diet, exercise routine or drug regimen prescribed by their physician. Крионирование удаленных органов

Сейчас в клиниках России огромному числу пациентов удаляют такое большое количество внутренних органов-по разным причинам, начиная от онкологии до.. .аппендэктомии и почками (при раке к-л сегмента почки). На данный момент мы не знаем как помочь таким больным кроме как удаление органа, но в будущем возможно будет возможность сохранить такие органы, не удаляя их, просто терапевтически и наименее инвазивно. Почка удаленная сейчас, может быть заморожена к примеру и например через лет 10 снова пересажена обратно в связи с ухудщением ф-ии другой почки. Все равно это будет дешевле чем выращивать новую в лабораторных условиях. Потенцильный объем рынка таких услуг сопостовим с гемабанком а может и больше...кроме того это бы увеличило инвестиции в крионику, а заодно и в геронтологию. Вы только представье сколько тонн органического материала от человека ежедневно вырезается, выбрасывается и проч...?

Клинический случай



Эта переведенная статья.  Она конечно требует небольшой коррекции и вообще я ее когда-нибудь может доперевожу. Но в целом можно понять суть. Сейчас же это уже не актуально.

Пациентка с аденокарциномой легкого и осложнением в виде синдрома Труссо, леченная антикоагулянтами и химиотерапией.
У 63-летней пациентки был диагностирован распространенный рак легкого, аденокарцинома, с осложнением в виде синдрома Труссо. Синдром влючал следующие симптомы:  тромботический асептический эндокардит , асимптоматический инфаркт головного мозга, тромбоз глубоких вен и ДВС (диссеминированное внутрисосудистое свертывание)слабой степени.  ДВС нарастало и множественная тромбоэмболия мелких  легочных сосудов привела к серьезному нарушению дыхания у пациентки.  Ей было выполнено переливание крови и проведена антикоагулянтная терапия с гепарином и растворимым рекомбинантным  человеческим тромбомодулином, снизившая проявление симптомов синдрома Труссо.  Также пациентке была проведена химиотерапия, которая привела к уменьшению размеров опухоли  и  исчезновению синдрома Труссо. Не существует разработанных схем ведения пациентов с синдромом Труссо. Интесивная  терапия  антикоагулянтами это метод, который  может быть эффективным способом  улучшения общего состояния таких больных, что позволяет в дальнейшем провети им комбинированную ПХТ.
Введение
Венозная тромбоэмболия это одно из частых осложнений у онкологических пациентов и особенно часто встречается у пациентов с раком легкого. Известно, что такое осложнение как тромбоэмболия, возникшее  вследствие злокачественной опухоли, называется синдромом Труссо. Это осложнение было впервые описано  в 1865году Труссо и обычно определяет плохой прогноз у таких пациентов. Клинические симптомы этого синдрома варьируют: от тромбоза глубоких вен до ДВС. Синдром Труссо определен как тромбоэмболия неустановленной этиологии, которая предшествовала постановке диагноза злокачественного новообразования внутренних органов или возникла при выявлении опухоли.  В настоящей статье, мы описываем случай возникновения синдрома Труссо у 63-летней женщины с аденокарциномой легкого.  У этой пациентки развилась тяжелая дыхательная недостаточность вследствие  легочной эмболии. Больная  была успешно пролечена комбинированной антикоагуллтерапией терапией и ПХТ.
Описание клинического случая
63-летняя некурящая больная поступила в клинику с жалобами на повышеную температуру тела в течение 4-х месяцев. Осмотр при поступлении не выявил никаких нарушений в состоянии, кроме субфебрильной температуры. По результатам лабораторных анализов выявилены следующие отклонения:  анемия, тромбоцитопения, нарушение процесса свертываемости крови и высокие значения онкомаркеров и уровня KL-6 антигена (Сиалированный углеводный антиген KL-6) см. табл. Предполагалось, что нарушение в свертываемости крови, было вызвано ДВС слабой степени.  Исходя из этих предположений пациентке была проведена длительная в\в терапия гепарином в дозе 10000МЕ в сутки.
В дальнейшем, пациентке было выполнено несколько лучевых методов исследования. На рентгене грудной клетки какие-либо отклонения отсутствовали. КТ грудной клетки с контрастом и ПЭТ выявили: 2,5см л\узел,  связанный с л\узлами корня нижней доли левого легкого, а также лимфаденопатию в средостении и левой подмышечной области. Так как у больной был заподозрен первичный рак легкого, ей провели трансбронхиальную биопсию левого прикорневого л\узла под контролем УЗ датчика.  Результаты гистологии и иммуногистохиимии – муцин продуцирующая аденокарцинома экспрессирующая TTF-1 фактор (тиреоидный фактор транскрипции 1). Результаты скринига на наличие мутации в рецепторе эпидермального фактора роста и реаранжировки гена ALK были отрицательные. Пациентке выставлен диагноз: аденокарцинома легкого T1bN3M1b (LYM- с мтс в л\узлы), IV стадии.
Учитывая тяжелое состояние пациента, вследствие нарушения нормального процесса свертываемости крови, ей дополнительно была сделана МРТ головного мозга. Она выявила множественные эмболии церебральных сосудов без каких-либо клинических симптомов. По Эхо-КГ был диагностирован асептический эндокардит с 1см тромботическими массами на передней и задней створках митрального клапана. Эти тромботические массы, вероятно были причиной множественной эмболии церебральных сосудов, выявленной на МРТ. Несмотря на наличие очевидных признаков тромбоэмболии на КТ с контрастом, в этот период , ТЭЛа у пациентки не была подтверждена. Таким образом, окончательный диагноз  был определен как аденокарцинома легного IV стадии с синдромом Труссо.
На 15 день пребывания в клинике у пациентки отмечалось ухудшение лабораторных показателей: тромбоцитопении и повышение уровня Д-димера. Согласно DIC score (шкале), состояние пациентки оценено в 10баллов, поэтому мы начали проводить в\в инфузию растворимого рекомбинантного человеческого тромбомодулина. Кроме того, дыхательная недостаточность больной быстро нарастала.  При повторном проведении КТ грудной клетки с контрастом не было выявлено очевидных признаков ТЭЛа. Тем не менее, больной сделана сцинтиграфия легкого, которая указала на нарушение кровотока в периферических сосудах легкого.  Причиной нарушения кровотока в периферических сосудах легкого стала множественная тромбоэмболия мелких сосудов, поэтому дополнительно больной мы начали проводить оксигенотерапию. Кроме того, учитывая наличие ДВС у данной больной, мы  вводили тромбоконцентрат и свежезамороженную плазму одновременно с постоянными в\в инфузиями гепарина и тромбомодулина. В результате лечения тромбомодулином  было отмечено частичное уменьшение ДВС синдрома, что подтвержалось постепенным повышением уровня тромбоцитов и снижением значения Д-димера до нормы. Однако, значительных  измений в общем  состоянии (Perfomance Status составил 3- я так понимаю это аналог ECOG) пациента не отмечено. 
Мы предположили, что отсутствие особых изменений в общем состоянии пациента обусловлено наличием синдрома Труссо и основным методом для умененьшения проявлений этого синдрома будет воздействие на опухоль.  Пациенту была предложена ПХТ, несмотря на то, что такая терапия была  рискована, учитывая общее тяжелое состояние пациента и возможные нежелательные побочные действия. Информированное добровольное согласие получено. На 22 день пребывания пациента в клинике, ему  было проведено 1 введение комбинированной химиотерапии по схеме карбоплатин (AUC 5) и пеметрексед (500мг\м2).  Всего было выполнено 6 курсов ПХТ.  Токсичность: тромбоцитопения 3 и 4 степени.  Получен частичный ответ на противоопухолевую химиотерапию в виде уменьшения размеров опухоли на 40%. Центр исследований молекулярных механизмов старения и возрастных заболеваний создается на базе шести лабораторий МФТИ

Впервые в России кластер университетских лабораторий объединяется в единый исследовательский центр для решения прикладной жизненной задачи — повышения качества жизни в пожилом возрасте и борьбы с возрастными заболеваниями.

Центр объединит три существующих и три новых лаборатории Физтеха, занимающихся изучением мембранных белков. В управление и наблюдательный совет Центра войдут представители международного сообщества ученых, занимающихся исследованием механизмов старения, в том числе основатель оптогенетики Эрнст Бамберг, биофизики Рэй Стивенс, Георг Бюлдт, Валентин Горделий, Вадим Черезов и другие.

Фонд «Сколково», IBM и Первый Онкологический Научно-Консультационный Центр подпишут меморандум о взаимопонимании



IBM (Watson), Первый Онкологический Научно-Консультационный Центр (ПОНКЦ)   и Сколково сегодня подпишут историческое соглашение, где две интеллектуальные системы смогут работать вместе в области персонализированной медицины и продления здорового долголетия.Проведение мероприятия запланировано в Петербурге в музее Фаберже (Шуваловском Дворце). Будет объявлен первый в истории Watson проект такого рода в присутствии В.Ф.Вексельберга, глав IBM и ПОНКЦ.

Российский бизнесмен Дмитрий Каминский пообещал наградить первого человека, достигшего возраста в 123 года, миллионом долларов CША и вместе с ним отпраздновать день рождения ради зарождения "гонки долголетия" среди стариков и повышения внимания общества к проблемам продления жизни, сообщает пресс-служба венчурного фонда Deep Knowledge Ventures.

"Многие пожилые люди говорят сегодня, что они устали жить. Это эволюционный и социальный феномен, который я хочу изменить. В последние годы мир все больше входит в гонку долгожительства. Я ставлю мое собственное здоровье и деньги ради достижения этой цели, однако все наши усилия будут напрасны, если нам не удастся убедить стариков стремиться к жизни", — заявил Каминский.

Who can get 1 million dollar after 123 years old?

In the last few years, major anti-aging companies, such as Google's Calico and J. Craig Venture's new San Diego-based genome sequencing start-up Human Longevity Inc, have launched.

Along side this, Billionaires like Larry Ellison, Sergey Brin, Peter Thiel, Paul F. Glenn, and Dmitry Itskov are also funding research into longevity science.

Itskov is the founder of the 2045 Initiative with the goal of helping humans achieve physical immortality within the next three decades.

Reuters reported that gerontologist Dr Aubrey de Grey, chief scientist at SENS Research Foundation and Anti-Aging Advisor to the US Transhumanist Party, thinks scientists may be able to control aging in the near future. 'I'd say we have a 50/50 chance of bringing aging under what I'd call a decisive level of medical control within the next 25 years or so,' he said. 

A Russian multi-millionaire whose dream it is to live forever has promised to give $1 million to the first person to reach the age of 123.
Dmitry Kaminskiy, a senior partner of Hong Kong-based firm, Deep Knowledge Ventures, is hoping his million dollar gift will trigger a new group of 'supercenternarians'.

He says research into stem cells, tissue rejuvenation and regenerative medicine will allow people to live beyond 120 - an age that has been quoted as the 'real absolute limit to human lifespan'. 'We live in the most exciting era of human development when technologies become exponential and transformative,' Kaminskiy told DailyMail.com. 

Kaminskiy is hopeful people will soon start living to 150 years of age with longevity science improving. Studies in stopping and reversing aging in mice have already shown some success, and people around the world are generally living longer all the time.

For example, life expectancy hit an all time high of nearly 79 years-old in 2014 in America according to a report from Centers for Disease Control and Prevention's National Center for Health Statistics. 

Additionally, Britain's oldest person, Ethel Lang, just died at the age of 114. She was believed to be the last person living in the UK who was born in the reign of Queen Victoria. Whether Kaminskiy's million dollar prize will encourage people to live 
longer is still to be seen.

Участниками спора стали - глава специализирующейся на изобретении медпрепаратов против старения биотехнологической компании Insilico Medicine Inc. Договор утратит силу в том случае, если оба участника спора доживут до ста лет. Также было оговорено условие, согласно которому Жаворонков и Каминский пообещали не способствовать смерти друг друга, то есть, не будут нанимать убийц, подстраивать несчастные случаи, травить друг друга и все прочее.

Напомним, Дмитрий Каминский и Алекс Жаворонков вместе с биологом Антоном Буздиным и биофизиком-математиком Николаем Борисовым занимались разработкой системы «Онкофайндер», которая позволяет подбирать препарат для лечения конкретной опухоли, а также может использоваться для поиска новых лекарств при лечении рака. Эта система не имеет аналогов в мире и сейчас проходит тестирование в нескольких российских онкологических клиниках.

NewScientist : Everyday drugs could give extra years of life


Drugs to slow ageing sound futuristic but they already exist in the form of relatively cheap medicines that have been used for other purposes for decades. Now that their promise is emerging, some scientists have started using them off-label in the hope of extending lifespan – and healthspan. "We are already treating ageing," said gerontologist Brian Kennedy at the International Symposium on Geroprotectors in Basel, Switzerland, last week, where the latest results were presented. "We have been doing ageing research all along but we didn't know it."

Last year Google took its first steps into longevity research with the launch of Calico, an R&D firm that aims to use technology to understand lifespan. Geneticist Craig Venter announced he is pursuing a similar goal via genome sequencing. Now pharmaceutical companies look set to join in. At the conference, the head of Swiss drug firm Novartis said research into "geroprotectors" or longevity drugs was a priority.

Google and Venter's plans may have injected an over-hyped field with a measure of credibility but they are unlikely to bear fruit for some time. Yet evidence is emerging that some existing drugs have modest effects on lifespan, giving an extra 10 years or so of life. "We can develop effective combinations for life extension right now using available drugs," says Mikhail Blagosklonny of the Roswell Park Cancer Institute in New York.
One of the most promising groups of drugs is based on a compound called rapamycin. It was first used to suppress the immune system in organ transplant recipients, then later found to extend lifespan in yeast and worms. In 2009, mice were added to the list when the drug was found to lengthen the animals' lives by up to 14 per cent, even though they were started on the drug at 600 days old, the human equivalent of being about 60.

This led to an explosion of research into whether other structurally similar compounds – called rapalogs – might be more potent. Now the first evidence has emerged of one such drug having an apparent anti-ageing effect in humans. A drug called everolimus, used to treat certain cancers, partially reversed the immune deterioration that normally occurs with age in a pilot trial in people over 65 years old.
Immune system ageing is a major cause of disease and death. It is why older people are more susceptible to infections, and why they normally have a weaker response to vaccines.

That weak response, however, has proved useful for studying ageing, as it provides an easy read-out of immune system health. "In humans you can't do decades-long clinical trials," says Novartis researcher Joan Mannick. Instead, the company looked at a proxy that would quickly show results.
They gave 218 people a six-week course of everolimus, followed by a regular flu vaccine after a two-week gap. Compared with those given a placebo, everolimus improved participants' immune response – as measured by the levels of antibodies in their blood – by more than 20 per cent, to two out of the three vaccine strains tested.

Of the three everolimus doses tested, the highest caused fatigue and mouth ulcers, while two lower doses had no apparent ill effects. Previous experiments in mice with rapamycin suggest this class of drug acts by inhibiting a protein called mTOR. mTOR also seems to be affected by calorie restriction – the strategy of trying to live longer by eating less.
mTOR is involved in sensing the level of nutrients available within cells, so one idea is that when times are scarce, cells shift into energy-conserving mode, which has knock-on anti-ageing effects, including on the immune system.

Mannick stresses that the study needs repeating, and the big question, of whether the drug keeps the participants healthier, can only be settled by long-term follow-up. There's also the issue of side effects beyond those seen in the trial. High doses of rapamycin used in organ transplants seem to nudge the recipient's metabolism into a prediabetic state – a harm that might outweigh its anti-ageing effect.
For now, it is an encouraging sign that rapalogs have similar effects in people as in mice, at least on the immune system, says Alex Zhavoronkov, CEO of biotech firm InSilico Medicine in Baltimore, Maryland.

Three distinguished experts have joined InSilico Medicine

InSilico Medicine proudly announces that Donald Small, MD, PhD, Kristen Fortney, PhD and Alexey Moskalev, DSc will aid the company’s mission to successful drug discovery and personalized medicine. It is clear meaningful progress is being made to battle aging from InSilico Medicine, considering the past major SAB additions.

Alex Zhavoronkov, CEO of InSilico Medicine, puts this exciting SAB addition into context,

Kristen Fortney is a Postdoctoral Fellow at Stanford University, where she works on the genetics of human longevity. She obtained a PhD from the University of Toronto. Her work consists of the application of computational biology tools to problems in translational medicine, focusing on aging and age-related disease. The technical guidance she will have to offer will play a large role in development and future goals of InSilico Medicine.

Dr. Moskalev, from the Institute of Biology of Komi Science Center of RAS is Head of the Laboratory of Molecular Radiobiology and Gerontology. He also teaches at Syktyvkar State University. Much of his focus has been on DNA repair and molecular mechanisms involved with aging. Dr. Moskalev’s leadership and global knowledge of aging research will play a key role in the delivery of InSilico Medicine’s solutions.
“While experiments in model organisms are extremely important for both cancer and aging research, advances in computing, availability of the vast amount of omics data and rapidly improving understanding of the underlying molecular processes allow for the many experiments to be performed in silico. The GeroScope system developed by Insilico Medicine at the very least allows to narrow down the large lists of geroprotective drugs to be tested in animals and for some drugs and combinations directly in humans. OncoFinder (Онкофайндер)

Российские ученые предложили уникальный биоинформатический инструмент для математического анализа внутриклеточных сигнальных путей под названием OncoFinder. 
Тех­но­ло­гию раз­ра­бо­тала меж­ду­на­род­ная команда под руко­вод­ством рос­сий­ских специалистов, в раз­ра­ботке и вали­да­ции при­няли уча­стие более 50 уче­ных из раз­лич­ных орга­ни­за­ций, в том числе группы Ни­ко­лая Бо­ри­сова (ФМБА Рос­сии), Сер­гея Ру­мян­цева (ФНКЦ ДГОИ, МФТИ), А­лек­сандра А­ли­пера (МГУ), Ан­дрея Га­ража (МФТИ), Ми­ха­ила Кор­зин­кина (МИФИ), Ольги Ко­валь­чук (Аль­берта, Ка­нада), Чарльза Кан­тора (Бо­стон­ский У­ни­вер­си­тет), кол­лек­тива Чин­сонга Жу (У­ни­вер­си­тет Джонс Хоп­кинс), А­лек­сандра Жа­во­ронкова (ФНКЦ ДГОИ, МФТИ).
Алгоритм OncoFinder может считывать и обрабатывать большой объем внутриклеточных сигналов, которые определяют все основные процессы в клетке: рост, дифференцировку, пролиферацию, миграцию, жизнедеятельность, превращение в опухолевую клетку и клеточную смерть. На основе полученных данных OncoFinder дает количественную и качественную характеристику активации сигнальных путей и сравнивает эти показатели с нормой.

Николай Владимирович Жуков, заведующий отделением клинической онкологии ФГУ Федерального научно-клинического центра детской онкологии, гематологии и иммунологии, доцент кафедры онкологии, гематологии и лучевой терапии РНИМУ им. Н. И. Пирогова:
«Пока в практической деятельности врачей-онкологов по-прежнему используются валидированные методики (то есть доказавшие свою эффективность в рамках клинических испытаний), основанные на иммуногистохимических тестах и определении отдельных мутаций. И это правильно, так как любой тест, на основании которого принимается решение о назначении или изменении лечения, влияет на судьбу больного не меньше, чем само лечение. И, соответственно, эффективность этого теста должна быть абсолютно доказана.
Но за тестами, подобными OncoFinder, безусловно, будущее. Все дело в том, что используемые в настоящее время методики весьма неточны. Да, их возможность предсказывать эффективность или неэффективность лечения доказана, но далеко не идеальна: у многих больных, у которых по результатам применяющихся в настоящее время методик ожидается достижение противоопухолевого эффекта, на практике опухоль не реагирует на лечение. Это связано с тем, что применяемые сейчас тесты «механистичны»: они анализируют лишь один признак опухоли, на основании которого выдают ответ – лечение поможет или нет. Но жизнедеятельность опухоли гораздо сложнее, выявление «мишени» для какого-либо вида терапии не значит, что эта терапия обязательно будет эффективна. Даже при блокаде «мишени» (чаще всего сигнального пути клетки) может оказаться, что в клетке есть другие сигнальные пути, которые с успехом замещают заблокированный. И опухолевая клетка продолжает жить и размножаться, убивая организм больного, несмотря на, казалось бы, прицельное (по факту наличия мишени) назначение препарата.
Методика же OncoFinder потенциально позволяет давать гораздо более точный прогноз, так как учитывает взаимодействие различных сигнальных путей: не только того, который планируется заблокировать, но и тех, которые могут взять на себя его функции. Кроме того, в отличие от генетических тестов, оценивающих «потенциал клетки» (то, что может быть задействовано в ее жизнедеятельности при реализации имеющейся в ней генетической информации), методика OncoFinder оценивает уже имеющиеся сигнальные пути (то, что реально активировано или инактивировано в клетке, что реально отвечает за происходящие в ней процессы и ответ на лечение). Пока эти данные являются лишь предварительными, для выхода в широкую клиническую практику методика должна пройти процесс валидации, однако отрадно, что группа именно российских ученых столь далеко продвинулась в данной крайне перспективной области, имеющей огромный потенциал развития. В случае подтверждения эффективности методики она будет иметь поистине революционные последствия во многих областях медицины, связанных как с лечением больных, так и с разработкой новых препаратов для лечения и профилактики различных заболеваний. Разрабатываемый аналитический аппарат не является сугубо специфичным именно для онкологии, он может быть использован в любой области медицины, требующей понимания внутриклеточных процессов и влияния на них различных методов лекарственной терапии.
